What We Do - Service Overview

THE STRUCTURED DAY CARE PROGRAMME – TIER 3

Aim

To provide an immediate and accessible provision, for the needs of a client’s substance misuse, health and social functioning for stable and abstinent Service Users. We aim to provide an intensive service treatment with rigid monitoring and tracking protocol to support movement and progress with the Service User through their treatment within a Structured Day Care Provision.

Client Group Criteria

Clients are eligible to access the Structured Programme having being assessed as needing further Structured Psychosocial Interventions.

The Structured Day Programme offers an intensive treatment package for the duration of a 12 week minimum programme. Kickstart does not operate holding groups.

This provision can be accessed by Service Users being Abstinent, Stabilised, in receipt of community prescribing, or Service Users assessed as needing structured Day Care Treatment as an option.

Clients can expect to be seen within 48 hours, and will commence service treatment within 2 weeks.

Referrals are accepted from the individual, DIP, the probation service, prescribing agencies and treatment services. We will make every effort to keep Clients engaged until referrals are picked up, either to or from the service.

The minimum age for access is 18 with no upper age restrictions.

Structure & Content

The 12 week programme will run from Monday to Friday 9 – 5pm on a structured weekly rota basis and include:

  • Triage Assessment
  • Comprehensive assessment
  • Risk Assessment
  • Motivational interventions
  • Care Plan
  • Client induction
  • Exit and Aftercare Plan

In addition to one to one Key– working (minimum of 2 per client per week), Clients will be offered more sessions on an agreed diary basis with their Key-worker. Flexibility is paramount for the success of the client / key worker relationship.

Care planned support from interview and assessment through to exit and aftercare / referral. Care plans are reviewed every month. One to one sessions work in conjunction with the care plan’s objectives, on a daily basis.

Every month's Aftercare Plan review is scheduled involving the Client, their Key-worker and all external agencies involved in the care of the Client to discuss and review their progress and treatment. This session can also involve family and children.

On construction of the Client's last care plan, an exit and aftercare plan is developed in preparation for a clear planned exit from the service within good time and support. This may involve external bodies in relation to their aftercare plan.

Clients can be referred to Structured Psychosocial Interventions external to the service as part of their care package and care planning. These have been commissioned by the DAAT from tier 2 and 3 provisions, Turning Point and Phoenix Futures.

Service Users will be able to access the Kickstart's evening support groups each week. These run on Monday and Wednesday evenings 4 – 7pm and offer a range of activities to include informed education and training, representation form City's service provision such as GUM, Family & Friends, Centre for HIV, training workshops, film evenings and recreational activities.

All clients will have a comprehensive assessment, reviewed care plans, weekly client reviews, regular one to one sessions, monthly Careplan review, exit and aftercare plan, closing report, and aftercare support.

Aftercare

At the last care plan and exit review an aftercare plan is constructed with the Service User and all external bodies involved in the Service User's aftercare and support. At entry to the programme clear criteria is given in relation to relapse and re-entry back to the programme or alternative support will be made available and is reviewed accordingly. This enables supported planned discharge from the programme.

Service User Representation

Kickstart participate in the DAAT Service User representation scheme. Service Users will have access to and be represented by Kickstart throughout the Structured Day Care Programme. This usually takes place on a Wednesday morning when the Service User representative attends the programme to facilitate the session.

Kickstart also has a Service User feedback booth situated on the premises for client access.

Capacity & Planning

Key-workers will facilitate the Structured Day Programme. The Service Manager will also deliver parts of the training modules.

The programme will run on a weekly rota, commencing each day at 10am. Clients can access the programme for one to one sessions and support from 9am.

We run a structured training programme every day including recreational activities weekly. Training workshops close at 4pm, allowing 4pm – 5pm for one to one support and actioning care plan activity etc.

Staff will deliver training and supportive interventions on a daily basis alongside Key–working an individual case load of clients. The Structured Day Care Programme shall run 4 times a year offering rolling access to the programme. The minimum group capacity is 15. We aim to achieve a minimum of 90% client retention for the duration of the programme. 100% will receive a discharged plan.
